2008 Lourdes Health System Golf Classic

Calling all Golfers!! Get into the “swing” and save the date for the Lourdes Health System Golf Classic. Levels of sponsorship, ranging from $450 to $25,000, are available and include participation benefits at the classic. Cost:
Golf: $450 per player*
Dinner & Cocktails Only: $100 per guest

*Player fee includes greens fees, golf cart, driving range, entry in skills challenge competition locker, continental breakfast, lunch, snacks on the course, cocktails, hors d'oeuvres and dinner.

Raffle Prize: Your choice of golf for 4 at one of the following destinations: Pebble Beach, CA, Pinehurst, NC, Doral Golf Resort, FL, Scottsdale, AZ or Los Cabos, MX.

Raffle Cost: $250 each (only 100 will be sold)

Format: Individual Low net, individual low gross, individual calloway, best ball of foursome

Deadline: Space is limited and registration is on a first come, first served basis. Early registration and sponsorship commitments are recommended!

Payment: Register via online or printable form. Golfer registration fees, sponsorships, and dinner tickets may be paid by check, money order, or credit card (MasterCard, Visa or AMEX). Checks should be made payable to Lourdes Health Foundation.
