The Rehabilitation Treatment Team
Our staff work together as a team to coordinate all aspects of an individual's recovery and to maximize the effects of treatment. The team is led by a case manager who is responsible for ensuring that an individual's treatment plan is carried out according to his/her goals and the goals of the family.The treatment team includes members from a number of disciplines.

The patient and family work as part of this team to help set and achieve mutual goals. We regularly hold meetings to discuss treatment goals and progress as well as to make recommendations. Patients and families are invited to attend these meetings, as requested or needed.
