Admission Criteria
Admission to the Comprehensive Inpatient or Outpatient Medical Rehabilitation Program requires:- Physician referral
- An approved rehabilitation diagnosis such as:
- A neurological disorder such as multiple sclerosis, Parkinson's disease, motor neuron disease, polyneuropathy, post polio syndrome or Guillain-Barre Syndrome.
- A stroke with functional deficit; brain injury; or multiple trauma.
- Paraplegia from spinal cord injury; amputation of a lower extremity; or complex orthopedic condition.
- Bilateral joint replacement or hip fracture.
- by managed care, morbidly obese or has a significant co-morbidity such as diabetic neuropathy or dialysis
- Limitations must be of recent onset or recent regression or progression, unless the individual has not had prior exposure to rehabilitation Expectation of achieving significant improvement in the individual's level of function within a reasonable period of time
- Medical conditions must be stabilized to allow individuals to participate in an active rehabilitation program.
- In addition to the above criteria, comprehensive inpatient rehabilitation candidates must be 20 years of age or older, be able to participate in a combination of at least three hours of physical, occupational, and/or speech therapy per day, require 24-hour intensive rehabilitation nursing care or supervision, and have an established discharge plan.
